Pocono Medical Home Care is now "at home" in new and expanded office facilities on the corner of Prospect and Ridgeway Streets in East Stroudsburg, PA.

When your doctor prescribes Home Care, you want to know that the health professional at your side is part of a dedicated team of home care specialists. You want the medical professional in your home to bring knowledge, experience and compassion with each visit.

Pocono Medical Home Care, Inc. accepts patients on a referral basis from physicians, institution discharge planners (hospitals, nursing homes, etc.), social service agencies, family members, responsible parties and patients as self referrals.

Upon admission, the patient is assessed and evaluated as to medical needs and a plan of treatment is established and periodically reviewed under the direction of the patient's physician.

Frequency of home visits by Pocono Medical Home Care, Inc. staff are established depending on the acuteness of the patient's condition and the care needed. Visits are scheduled as ordered by the physician per the plan of treatment.

Pocono Medical Home Care, Inc. is licensed by the Commonwealth of Pennsylvania and is Medicare certified. Services are billed directly to Medicare, Medicaid, Blue Cross, Private/Commercial Insurance and other types of insurance and provider programs. Patients may also pay privately for services not otherwise covered.

Important guidelines for Medicare/Medicaid and most other insurance programs:

1. The patient must be homebound, requiring considerable effort and assistance to leave their home.
2. The care must be medically reasonable, necessary and ordered by their physician.
3. The care must be skilled, part-time and intermittent in nature.

It is the policy of Pocono Medical Home Care, Inc. to admit and treat patients without regard to race, color, national origin, ancestry, age, sex, religious creed, handicap or disability.

There is no distinction in eligibility for, or in the manner of providing, any service provided by Pocono Medical Home Care, Inc.
